Location:  Costa Fields at Smyrna Park

 

Time:  9 am—2 pm

 

Directions:  From Highway 99, Exit Hatch Rd & go East, Right on Moffett Rd, Left on Fowler Rd, Right on Rose Ave., baseball fields are to your right.

REGISTRATION DAY 05-03-2008 & 05-04-2008

 

 

Location:  Ceres Street Faire

 

Time:  10:00 am—4 pm

 

Directions:  We will be located between Whitmore Park and the Ceres Fire Department.

Registration

How to Register

 

You can register your child(ren) by visiting us at one of our registration events listed below or you can download the CYSA Membership Form 1601.  Deadline is 6/1 and envelopes must be postmarked by 6/1 to be eligible.

 

 

 

 

 

 

Submit the Following:

 

                 1)  The original CYSA Membership Form 1601 (make sure

                       that the parent or legal guardian signs in the indicated area with

                       wet ink AND provide the mother’s birthdate).

                 2)   A copy of the original 1601 after it has been filled & signed.

                 3)   A copy of the birth document (A hospital certificate is not acceptable).

                 4)   A check or money order payable to CYSO.

Cost

 

                 $  75 1 player

                 $140 2 players

                 $210 3 players

                 $250 4 players

 

To qualify for multi-player discounts, players must be siblings and payment must be received for all players at the same time. 

 

 

Verification of Birth

 

A copy of the birth document is required for all new players during registration.  Hospital certificates are not accepted.

 

 

Age Requirement

 

Players who turn four years old on or prior to 07-31-2008 are eligible to play.  For example, players born between 08-01-2002 through 07-31-2004 will be placed in the under 6 division.

 

 

Waiting List

 

Last day to register your child(ren) will be 06-01-2008.  Registrations received after this date will be placed on a waiting list.  The waiting list is not a guarantee that your child(ren) will be placed on a team.  If we cannot place your child(ren) on a team, then you will be refunded the registration and late fees.

 

To be on the Waiting List:  Mail the 1601 registration form, copy of the birth document, and payment (including a $10 late fee per player).

 

 

Late Fee

 

Please include a late fee of $10 per player for late registration.

 

 

Refund Policy

 

No refunds.

 

 

Practices

 

Practices should begin after August 1st.  If you have not heard from your coach by August 15th, please contact us.

 

 

Games

 

Games will begin on 09-06-2008 and continue every Saturday for a total of ten weeks.

 

 

Picture Day

 

Picture Day will be on 08-23-2008 at Smyrna Park.  Times are listed below.

 

 

Picture Make-up Day

 

Picture Make-up Day will be on 09-18-2008 from 5:30-7:00 p.m. at Smyrna Park.
